To be eligible for inclusion in the competition, this entry form must be accompanied by:
- Five copies of your 2-4-page essay (one copy with your name on it, four without)
- Five copies of a formal bibliography (one copy with your name on it, four without). For the bibliography format, follow an established style guide.
- A sampling of books from the collection (3-5 books)
